* Please put WAKG at the top of your entry and we must receive it at least three buiness days in advance, but no more than 30.
* Please print and use a regular size piece of paper.
* One birthday per page.
* Please include birthdate, name, address and phone number.
* This is a contest and we cannot accept entries over the phone; however, if you'd like to call during our office hours, the receptionist would be happy to check and see if we've received your entry.
